- Active perianal sepsis
- Uncontrolled coagulopathy
- Active inflammatory bowel disease flare involving the anorectum

- Early Ambulation — ERAS pillar — walking within hours of surgery prevents complications.
- Deep Breathing & Coughing — Prevent postoperative pulmonary complications.
- Diet Advancement — ERAS encourages early oral intake.
- Daily Pain & Symptom Check-In — Track pain, energy, mood, and any new symptoms to detect complications early.
- Wound / Incision Inspection — Check incision sites for infection (redness, drainage, dehiscence).
- Hydration & Nutrition Log — ERAS emphasizes early oral intake and adequate hydration.

- Heavy bleeding (more than spotting, especially with clots)
- Severe pain not controlled by prescribed medications
- Inability to urinate for 8+ hours post-operatively (urinary retention)
- Fever above 38.5°C with worsening perianal pain (perianal sepsis)
- Severe constipation with no bowel movement for 5+ days despite stool softeners
- Loss of bowel control
- Loss of consciousness or near-syncope
- Signs of significant blood loss (dizziness, pallor, tachycardia)
