• Exposed major blood vessels (risk of erosion and hemorrhage)
• Active malignancy within the wound bed (relative; palliative use may be considered)
• Non-enteric or unexplored fistulae to organs or body cavities
• Necrotic tissue with eschar requiring debridement before NPWT application
• Known allergy to drape adhesive or sponge material
