- Inability or unwillingness to adhere to lifelong immunosuppression
- Severe, unstable cardiopulmonary disease without optimization

- Daily Pain & Symptom Check-In — Track pain, energy, mood, and any new symptoms to detect complications early.
- Wound / Incision Inspection — Check incision sites for infection (redness, drainage, dehiscence).
- Hydration & Nutrition Log — ERAS emphasizes early oral intake and adequate hydration.

- Sudden drop or stop in urine output, or rapid painful swelling over the transplant site (possible rejection, clot, or urine leak)
- Fever over 38C (100.4F) with chills while immunosuppressed (infection can become life-threatening quickly)
- Severe abdominal pain over the new kidney with firmness and tenderness
- Chest pain, severe shortness of breath, or one-sided leg swelling and pain (possible clot)
