- Suspected testicular malignancy (requires inguinal orchiectomy, not hydrocelectomy alone)
- Uncontrolled coagulopathy

- Fever above 38.5°C with scrotal redness, warmth, and spreading skin changes
- Severe pain unresponsive to prescribed medications
- Signs of wound dehiscence with exposed tissue
- Signs of systemic infection (fever, chills, confusion)
