- Active untreated urinary tract infection (relative; treat first)
- Acute urethral injury or recent urethral surgery

- Heavy bleeding with clots that block urination (clot retention)
- Fever above 38.5°C with chills and flank pain (urosepsis concern)
- Severe abdominal or pelvic pain suggesting bladder perforation
