- Fever above 38.5°C with chills and flank pain (urosepsis concern)
- Severe abdominal or pelvic pain suggesting bladder perforation
- Persistent hematuria beyond 3 days that is not improving
- Increasing dysuria or new pelvic pain after initial improvement
- Cloudy or foul-smelling urine suggesting urinary tract infection
- Low-grade fever (38.0-38.5°C)
- Persistent urgency and frequency beyond 5 to 7 days
