- Unrecognized pharyngocutaneous fistula requiring surgical management
- Flap or wound breakdown requiring return to the operating room
- Inability to safely manage the stoma at home without adequate support or training

- Saline Rinses / Spray — Keep nasal/oral mucosa moist and clean.
- Soft / Cool Diet — Reduce trauma to surgical site during eating.
- Bleeding Watch — Detect post-tonsillectomy / sinus bleeding early.
- Daily Pain & Symptom Check-In — Track pain, energy, mood, and any new symptoms to detect complications early.
- Wound / Incision Inspection — Check incision sites for infection (redness, drainage, dehiscence).
- Hydration & Nutrition Log — ERAS emphasizes early oral intake and adequate hydration.

- Sudden heavy bleeding from the stoma, mouth, or neck wound
- Saliva or food draining through the neck incision (possible pharyngocutaneous fistula)
- Rapidly spreading neck redness, swelling, or severe pain with fever
